Objective: Health information-seeking behavior is a multidimensional phenomenon influenced by various variables. The present study aimed to identify, screen, and prioritize the cognitive and contextual factors related to health information-seeking behavior from the perspective of experts.
Methods: This study was conducted using a mixed-methods approach (quantitative and multi-criteria decision-making). In the first phase, 28 factors extracted from the scientific literature were evaluated and screened by an expert panel using the fuzzy Delphi technique in two rounds (acceptance threshold of 0.60). In the second phase, the confirmed factors were ranked using the MARCOS technique based on three criteria: "expert specialty," "intensity of importance," and "evaluation uncertainty."
Results: The results of the fuzzy Delphi led to the elimination of four factors (e.g., severe fear and neuroticism) and the acceptance of 24 factors, 23 of which entered the MARCOS matrix. The final ranking revealed that clinical and cognitive components had the highest relative priority. Specifically, "perceived health threat" (0.749), "severity of symptoms" (0.746), "health literacy" (0.730), "desire for health control" (0.727), and "ability to identify credible sources" (0.724) ranked first to fifth, respectively. In contrast, technological, social, and demographic variables (e.g., social support, education, income, and age) were placed at the bottom of the ranking.
Conclusion: In today's information ecosystem, health information-seeking behavior, rather than being merely a function of technological facilities or socio-economic status, is an immediate response to psychosomatic (clinical) tensions and strongly relies on the user's processing (cognitive) capacities. Accordingly, designers of health information systems should shift their focus from developing physical infrastructure toward designing platforms sensitive to clinical urgencies and enhancing health literacy.
